Baby Killed By Kaiser Medication Error
Authorities are investigating a medication error that led to the death of a baby at Kaiser's Santa Clara hospital. The article cites three medication error deaths at Kaiser Santa Clara since 2004 - this number seems low to me, so I'm wondering what Kaiser does to re-catagorize other deaths. Here's another article.

To give an example of how far Kaiser is willing to go, check out the case of Lehna Jordann Brewer, in which Kaiser pursues evidence manipulation and cover ups galore. Lehna's father recently spoke out about Kaiser's HealthConnect debacle - the L.A. Times article mentions problems caused by outages in the Neonatal ICU.

In other news, Kaiser is dumping Martinez for the more upscale suburbs of Walnut Creek and Antioch. Kaiser's Modesto project is waiting on a full deployment of HealthConnect. That's going to be a loooong wait - lol. The California Deptartment of Managed Health Care still claims to be investigating the problems with Kaiser's EMR.

Kaiser Thrive has also posted a visual aid for Kaiser nepotism. Among the many complaints about Kaiser HR is they look the other way as high level managers hire their friends and family.

ROTFLMAO: For some unexplained reason, Kaiser has rejected my application for Culture Change Leader. :D

It's shocking how easily Kaiser Permanente manipulates the media about things like this. They call it a "medication error" as if that absolves them of any responsibility. Three people having died at a single facility in this amount of time due to improper dispensing or incorrect administering raises some big questions, especially given they admitted the first two were due to inadequate policies and procedures. So suddenly those policies and procedures are working perfectly, but, still, an infant dies?

There are reasons why most hospitals have a "check twice" policy for both dispensing and administration. It sounds like, in this case, there wasn't even a "check once" procedure. According to the article, KP claims it was a dispensing error, not an administering error. Inherently, there were mistakes on both the part of the pharmacy staff and the nursing staff if the dispensing error was not caught at the time the medication was administered to the infant.

I wonder if you might be able to dig deeper into this story. There have been several unreported "medication error" deaths recently at hospitals where KP has gone live with HealthConnect Pyxis integration. I hope more people don't have to die before the DMHC or someone (anyone?) starts demanding honest answers (not spin!) from Kaiser Permanente regarding the safety of HealthConnect.

West LA Inpatient Phase 1 was implemented last month. Anaheim is this month, followed by LAMC in the April timeframe.....

I don't think the DMHC investigation is doing anything to slow down the roll out of HealthConnect.

The DMHC is just paying lip service to "investigating" as usual. Their idea of investigating is calling up someone at Kaiser, asking them to provide their (made up) side of the story, and then going out for a three martini lunch.

Resignations

Mass resignations from the Kaiser Board. Kaiser's trying to keep it quiet for now. Can't name names. Four seats now empty. Maybe more before the dust settles? The chairman of the audit and compliance committee was forced to quietly resign. Did he know too much?

Wow, thanks. I'll be looking into this! The auditor resignation is probably related to Kaiser's repeated failure of the California Worker's Comp audits. Kaiser is years behind on claims, and they are in danger of losing their self-insured status.

OK, this is interesting. Wondering if the not for profit status of Kaiser Health Plan and Hospitals requires them to be open about the Board of Directors? I think I'll make it my business to find out...

I tried calling their main number. They said the information was on the website. When I asked if the list was up to date, they said it "might not be" and that they weren't sure when it would be updated again. So Kaiser "might not" have accurate information on its website.........

Any word yet? I think there are a lot of people who wonder and worry about who's running Kaiser, but they're about as "transparent" as mud, and they like it that way!

At least I'm only grieving the loss of my health and the children we'd planned to have, not a dead baby.

I can't look at that picture without crying. We did lose one, but at 20wks. I didn't see him, he'd already been gone a few weeks, and I was afraid of what I'd see. I can't even BEGIN to get my mind around that experience, although that picture feels too close for comfort.

There are times I just wish I could be one great big sword of karma aimed right at Kaiser. Except there's no sword big enough.

Nepotism at Kaiser

As a former employee of Kaiser, I can attest to the nepotism which adversely impacts the hiring of competent employees. I was in a position in which I was seeking to hire an administrative assistant, for a patient safety-related area of operations, at a Kaiser hospital. The geographical area of the hospial was where there was an extremely high unemployment rate. One would think that I would be getting large numbers of qualified applicants. I was forced to go through Kaiser HR for the hiring process. I only received a tiny number of applicant packets from HR, most of whom, it turned out were either family members or friends of current Kaiser employees. None of them were even remotely qualified for the work! Since this directly affected the safety of our patients, I started to probe into the HR process, and discovered they were screening out numerous candidates who were qualified, in favor of only sending me family and friends of Kaiser staff. When I did locate a qualified applicant outside of the HR department, and had her submit an application, HR went out their way, with every thinkable maneuver, to stall and make it impossible for the candidate to be hired.

I apologize for taking so long to approve your comment. I've been sick for several weeks, and have fallen behind on even the things I regard as paramount.

I agree that 95% of the problem is HR. They use arcane job descriptions to justify screening people out, and then they can let the "special" candidates slip in because "no one with the right qualifications applied." HR also coaches managers on how to submit dishonest documentation so they can do whatever they want, ethical or not. HR is structured simply to please managers at any cost, so they do many sleazy things.

I caught this early on Sunday morning as a replay on the radio.

As I listened early on it sounded not to bad. Then I realized it was
Halverson as the speaker. When the Q&A session arrived - it just made me sick. Listen to him shine on about Kaiser's wonderful transplant program and blame UNOS for the Kidney Transplant center fiasco. Makes me sick. Then he portrays HealthConnect as the best thing for the 21st century...

My ax to grind with Kaiser - my husband's now starting his 7th year on the Kidney list. In May and July of 2006, the Kaiser transplant surgeon turned away
kidney's offered to him by UNOS by saying he was "no longer interested". Thanks to our lawyer we now know he came up on the list 6 times. The transplant surgeon liar was none other than the same one that also tried promote the death of a donor not so long ago. Simply I hate Kaiser - they have ruined my family for the last 10 years.

I would like to thank you for this blog. I only found it on the report about your "blogger" conference on PharmaBlogSphere. I have worked for KP and GHC another KP enterprise. I can honestly say, in response to this post that KP KILLED my father and my uncle due to medical errors. I have pictures of my dad's chart, before they could take it away. I work in hospital QA.

I will release their names to you off line and how they HID their medical errors. They have many tricks to help with their benchmarking..

I have low back pain and so i been seeing my healthcare provider for it and he give me morphine for it and it cause me not to be able to go pee and so i called the advice nurse and she said that she will make a note of it and my doctor will call me tomorrow its was on the weekend so today came around 9:30 am i called them and they said the doctor does not think the new pills cause me not to be able to pee and that it was a hole diffirent deal to make a long story short kaiser is the worest f-ing healthcare company on the planet earth and i hope they go under soon so people can be treated the right way like they mean something and not just a number on a card

I am glad to have found this blog...I have an ongoing beef with KP re: "reasonable accommodation". You think it should take 2 years to get accommodations for low vision?
